ZOSTAVAX PROTOCOL

• Patient must have prescription from their doctor.
   
• We run the clinic on Wednesdays (by appointment only) from 4:30-5:30 pm.
   
• There is a nurse on premise to administer the vaccine.
   
• Cost is $209.95 (We will try to run through insurance).
   
• There is also a $20 Administration fee for the nurse. D
   
• Doctors can fax the prescription to us at 978-475-1662
   
• Patient will be called on the previous Tuesday to confirm the date.
